Are you a current client?
- NO – Please review our Fees Summary and complete our Online Collection Agreement
- YES – Please provide your client # with all claim submissions (continue reading below)
The process for submitting claims to our office for collection action is very simple! For each claim, please provide supporting documentation, including but not limited to:
- All available consumer demographic and claim summary information (download a fillable Account Submission Form or use the Online Account Submission Form for this purpose)
- Itemized invoice(s)/statement(s) of amount owed
- A signed office financial policy form or agreement which specifically states the consumer is responsible for any collection and/or attorney fees
- If the consumer is a minor (<18 years of age), please include any signed registration/intake form(s) indicating whom is legally responsible for amount owed
- Any written correspondence between you and the consumer
- For claim(s) already in the court system, any/all relevant legal documentation and attorney information
- Any other helpful information to assist with collection efforts
That’s it!!! Once in our system, a New Account Acknowledgement Form will be issued shortly thereafter to confirm receipt.
