Submit Your Claims

To submit a claim(s) into our office for collection proceedings, the process is very simple! We recommend either emailing or faxing to us the following documents:

• An Itemized statement of the amount owed
• All demographic information that you may have on the consumer(s).
• If the consumer signed an office financial policy form, or an agreement, that states they would pay any collection fee(s) please send that signed form to us as well when you submit the new claim into us.
• If the consumer is a minor (under 18 years of age) please include with the new account any signed registration/intake forms so we can determine whom is legally responsible for the amount owed.

That’s It !!! We will then enter the new account into our DebtMaster computer system and then email you or fax you back an acknowledgment from our software application shortly thereafter.
